I hereby appoint Ngwane Goldberg Resources to act on my behalf in respect of the position and company listed as follows:
I further state that no other agency has obtained permission from me to introduce me to this position at this client, and that to the best of my knowledge no other person has an interest in placing me in this position/company. I further acknowledge that I have not submitted my CV to, or applied for any position at this company in the last 12 months.
I acknowledge that Ngwane Goldberg will be entitled to a fee from the company in respect of my placement to the position listed, or to any other position offered to me by this company within 1 year from today’s date.
I further undertake to declare the interests of Ngwane Goldberg to the company at the time of my interview with the company.
Ngwane Goldberg has outsourced the following checks (as mentioned in the following paragraph) to their services providers in order to obtain details.
I expressly give Ngwane Goldberg permission to use my personal information as provided by me, whether in person, per e-mail or per info stated in my CV, including reference checks, background checks, work history checks, qualifications verification and criminal checks, Identity and/or passport number, date of birth and age, phone/cell numbers, email addresses, physical address, gender, race, sex and ethnic origin, photos, voice recordings, biometric data, marital/relationship status and family relations including emergency contact names and numbers, religious or philosophical beliefs, culture, personal and political opinion, salary information, education information, physical and mental health information including disability, medical history, blood type, membership to organisations/unions, personal opinions, views or preferences and language for the purpose of employment at its client.
This statement is made of my own free will, and I acknowledge by signature hereto that in the event of my appointment to the above mentioned company, that Ngwane Goldberg are in my opinion the “effective cause” of my appointment.
I hereby give my consent for Ngwane Goldberg to acquire a criminal check, to view subsequent check as well as share this with their client.
